INCIDENT_OFFNDR_DSPLNRY_ACTIONTemplate Description
This template is used to track disciplinary sanctions as well as referral to or participation in remedial programs.
- Each known offender should have one and only one sanction record identifying the "highest" or most severe sanction (Disciplinary Action Code between
S1andS10). - If referred to or participating in remedial programs, the offender will also have one or multiple additional records with Disciplinary Action Code between
R1andR9.
Template Specifications
1 - SUBMITTING AUN
The unique, 9-digit AUN assigned by the PDE.

2 - INCIDENT ID
An identifier assigned by the LEA to the incident; this identifier must be unique within each LEA per school year.

3 - OFFENDER ID
An identifier for the offender involved in an incident. Must be the same value used in Field 2 (PERSON ID) within the Person Template.

4 - DISCIPLINARY ACTION CODE
The code of the sanction taken against the offender for the incident or the remedial program the offender has been referred to or is participating in.
See Appendix AN in Volume 2 of the PIMS User Manual for a complete list of valid values.
5 - SCHOOL YEAR DATE
A single date (June 30), the last day of the current school year indicating the whole school year.

8 - ORIGINAL DISCIPLINARY ACTION DURATION
The number of school days the student was suspended, expelled, or removed to an interim alternative education setting for the incident. If the duration extends beyond the last day of the school term, report only the number of days up to and including the last day of the school term. If the entire duration of a disciplinary action S3- S9 is to be served in the following school year, leave blank and report S10 (other) and describe the actual disciplinary action and duration in field 11, DISCPILINARY ACTION COMMENT.

10 - RECEIVED SERVICES INDICATOR
A Y/N indicator of whether the student received educational services during expulsion.
