Template Description
This template connects one or multiple offenders to each specific incident. If there are multiple offenders include multiple records within this template with the same Incident ID.
Template Specifications
1 - SUBMITTING AUN
The unique, 9-digit AUN assigned by the PDE.

2 - INCIDENT ID
An identifier assigned by the LEA to the incident; this identifier must be unique within each LEA per school year.

3 - OFFENDER ID
An identifier for the offender involved in an incident. Must be the same value used in Field 2 (PERSON ID) within the Person Template.

4 - SCHOOL YEAR DATE
A single date (June 30), the last day of the current school year indicating the whole school year.

5 - OFFENDER TYPE
The type of offender.
Business Rules
When the offender is a student from another LEA and the student has an IEP, report as 2 - student with IEP.

6 - AGE AT TIME OF INCIDENT
The age of the offender at the time of the incident.

7 - GRADE LEVEL CODE AT TIME OF INCIDENT
The grade level of the offender at the time of the incident.

Required if grade is known. If unknown, leave null or blank.

8 - LLE NOTIFIED INDICATOR
A Y/N indicator that specifies whether the incident in which the offender is involved required notification (for police action, investigation, arrest, etc.) of local law enforcement (school, municipal or State Police), including a School Resource Officer. This does not include mere consultation with or notification to School Police Officers or local law enforcement without engagement (police action, investigation, arrest, etc).

The name of the local law enforcement office contacted regarding this offender and incident.

10 - ARRESTED CODE
An indicator of whether the offender was arrested for this incident; see Valid Values.
Note
Removal of student by police does not constitute an arrest. There will be many situations in which law enforcement takes a student into custody but will not initiate criminal charges against the student. Any initiation of prosecution is considered an arrest, whether or not the offender is physically taken into custody. This includes the filing of a juvenile petition, the filing of criminal charges, or the filing or issuance of a citation.

11 - ADJUDICATION CODE

17 - INJURY SEVERITY CODE
Specifies whether the offender sustained a physical injury.

21 - SENDING DISTRICT OR CHARTER AUN
Provide the AUN of the Charter school or District of Residence who sent the student to be educated at your LEA. If student is Foster, provide AUN of the District where foster parents reside.

Required if Incident Offender Field 5 (OFFENDER TYPE) is in 1, 2, or 3. Only AUNs of SD and CS are valid.

22 - NONEDUCATIONAL AGENCY PLACEMENT
At the time of the incident, was the student placed in a residential facility or group home by a non-educational agency (e.g., court systems; departments of corrections; departments of children, youth, and families; department of social services; etc.
