Template Description

This template defines the primary details of a discipline incident. One record should be submitted per each reportable School Safety incident. See the Discipline Domain description for more information on reportable incidents. If a school has no reportable School Safety incidents for the entire school year a record indicating this must be entered in the Location Fact Template, refer to that section.

Reporting Note

If a school has no reportable incidents for the school year, submit an appropriate record in the Location Fact Template indicating zero incidents for the reporting period.

1 - SUBMITTING AUN

The unique, 9-digit Administrative Unit Number (AUN) assigned by the PDE.

A single date (June 30), the last day of the current school year indicating the whole school year.

An identifier assigned by the LEA to the incident; this identifier must be unique within each LEA per school year.

5 - SCHOOL NUMBER OF INCIDENT

The PDE defined 4-digit code identifying the school where the incident took place.

8 - INCIDENT TIME FRAME CODE

Business Rules

Required if Field 7 (INCIDENT PLACE CODE) = 1 (On school property/grounds).

1 – before school hours 2 – during school hours 3 – after school hours


9 - INCIDENT AGAINST PROPERTY INDICATOR

A Y/N indicator that specifies whether the incident was against property.

24 - UNFOUNDED INDICATOR

The incident number used by the local law enforcement agency to identify the incident.
